Health Care LA Trainings and Attestation Forms
If directed to this site, please download the Health Care LA New Provider Trainings and Attestations in the link listed below. Completion of the trainings are required as part of the enrollment process with Health Care LA. Once you have reviewed the attached documents, please sign and date all Attestation forms and email the completed forms to jefmartin@eisnerhealth.org.
